| G.S. Levine's Workers’ Compensation Claim Consultants discovered
that a State Fund standard claim reserve had not changed in three
years. When the adjuster was questioned on the claim status, it was
determined that the claim should have been closed. This was an error
on the part of the insurance company and the Bureau was notified.
The result was a correction to three policy year experience modification
rates resulting in a total decrease of 12 points and more than $24,000
in premium savings. |
GS Levine's Workers’ Comp Claim Consultant monitored a case
involving a client's employee who had been indicted for Workers' Compensation
fraud. In spite of this, the insurance company continued to report
the initial claim reserves to the Bureau, adversely impacting our
client's Experience Modification rate. Through many discussions with
the insurance company, the reserve was reduced by more than $30,000
and the Bureau was notified of the insurance company error. The Experience
Modification correction is being adjusted and this will result in
premium savings for two policy terms. |

| A Worker's Compensation additional premium audit was reviewed by
the G.S. Levine staff. The audit included duplicate claim handling
charges, resulting in an erroneous additional premium of more than
$180,000. A correction was made prior to the audit ever being seen
by our client. |
After a client had their Workers’ Compensation Final Audit,
we reviewed the audit worksheets and source data documents. In the
review of the source document provided by the payroll service, it
was noted that the Overtime Wages for the year had actually been understated.
Once this correct had been made, it resulted in a refund to the client
of about $23,000. |
